Licensed Physical Therapist

Full time positions are available which includes an above market salary, company car, weekly bonus, and benefits. PRN positions are also available allowing you to choose your area, schedule, and the amount of patients you want to carry.

Registered Nurse

PRN positions are available allowing you to choose your area, schedule, and the amount of patients you want to carry. Home health experience required.

Licensed Physical Therapist Assistant

Licensed Vocational Nurse

PRN positions are available allowing you to choose your area, schedule, and the amount of patients you want to carry. Home health experience required.

Home Health Aide and Certified Nurse Aid

PRN positions are available allowing you to choose your area, schedule, and the amount of patients you want to carry. A minimum of 12 months in the last 2 years of home health experience required.

LVN In-Take Coordinator - Full time

Coordinate patient care between doctors and clinicians. Process doctor orders. Track Podiatry, MBS, Diagnostics, INR results, wound care, labs and UI. Coordinate lab draws with clinicians. Intake patient referrals.

Education Director RN - Full Time

Strong Clinical background, home health with OASIS-C Knowledge, and recent educator experience preferred.

Admissions Coordinator - Full Time

Requires the ability to quickly establish rapport, credibility and integrity with existing accounts and pending prospects.  Develop existing relationships into steady referral sources and perpetually research territory for high-return prospects, identify decision makes and get focused face-time with same.  Effectively articulate our value as it applies uniquely to each prospective referral source.  Must thoughtfully approach every day and every visit to maximize potential return and maintain (and raise) the values, standards and culture of this company at all times.  Verbal report to ownership and teammate’s twice weekly and hit monthly quota within 60 days. Must have reliable transportation.
